The Flexible CBT Approach

The Flexible CBT Approach is based on Dr. Neuhaus’ Fixed Values and Flexible
Treatment model which adapts essential aspects of cognitive behavior therapy (CBT) principles and evidence-based literature into a pragmatic, flexible approach. Designed for patients with mood, anxiety, and personality disorders, and for those with severe and complex psychopathology, the treatment mission is to diminish a patient’s suffering, improve functional capacities, and enhance quality of life and interpersonal connections.
Atheneum Learning’s TRi Training System puts this approach into action in a series of dynamic, online training modules.
Module 1 – Fixed Values
Conceptual grounding for the treatment framework providing comprehensive foundational knowledge. There are seven fixed values organized according to two areas:
I. Treatment Framework
- CBT Triangle
- Functional Improvement
- Skill Acquisition
- Collaboration and Connection
II. Background Assumptions
- Evidence-based CBT treatments are the foundation of the approach
- Description of Life Context is a Therapeutic Process
- Treatment is a Learning Process that progresses in stages
In Modules 2 and 3, Fixed Values are operationalized into Principles and Protocols for applying the Flexible CBT Approach. Principles are the anchor points to guide clinical decision-making in assessment and treatment. Protocols are specific methods to apply. Knowledge of principles and protocols facilitates rigorous clinical judgment leading to effective treatment.
Module 2 – Assessment and Beginning the Work of Therapy
Introduction to the Cognitive Behavior Analysis (CBA) for patient assessment, and a framework for establishing an effective therapeutic relationship.
I. Establishing the Relationship
- Therapist perspective: Create structure, instill hope, stay connected
- Patient perspective: Going to therapy is nosmall thing for most people – anxiety, trepidation
- Setting expectations collaboratively
II. Cognitive Behavioral Analysis (CBA)
- Integration of patient and therapist perspectives
- Principles/Protocols: ‘Why now?’ questions; using the triangle; description of life context; racial and cultural factors; diagnosis/mental status
- Case formulation leading to treatment planning
Module 3 – Skills Training & Psychoeducation
From clinical formulation to interventions.
I. Bridge between CBA and Skills Training
- Principles/Protocols for core CBT skills
- Collaborative psychoeducation
- Stages of the therapeutic process
II. Learning to change — accepting changes made
